Ohio Police & Fire - New Disability Benefit Application Form

On September 24, 2013, the Ohio Police & Fire Pension Fund redesigned the Disability Benefit Application. Effective October 1, 2013, all new Disability Benefit Applications submitted to OP&F must be completed on the new form. OP&F, however, is allowing a sixty (60) day grace period, where they will accept the "old" Disability Benefit Application. As of December 1, 2013, OP&F will not process any Disability Benefit Application submitted on an old form.
